As a key member of the Primary Healthcare Team, you play a pivotal role in delivering comprehensive care to individuals of all ages within General Practice (GP) surgeries. You ensure that individuals receive high-quality healthcare services tailored to their unique needs.
Responsibilities:
Assessment and Screening: Conduct thorough health assessments and screenings for individuals across the lifespan, from infants to older adults. Identify health risks, detect early signs of illness, and provide evidence-based interventions to promote health and prevent disease.
Treatment and Management: Provide a wide range of clinical treatments and interventions to address acute and chronic health conditions. Administer medications, dress wounds, perform vaccinations, and manage long-term conditions, such as diabetes and hypertension, in collaboration with the healthcare team.
Health Promotion and Education: Deliver health education and promotion initiatives to empower individuals and families to make informed decisions about their health. Offer guidance on lifestyle modifications, preventive care measures, and self-management strategies to optimize health outcomes.
Child and Adolescent Health: Provide specialized care to infants, children, and adolescents, including well-child checks, immunizations, developmental assessments, and adolescent health education. Collaborate with pediatricians and other healthcare professionals to support the healthy growth and development of young patients.
Women’s Health: Offer a range of women’s health services, including cervical screening (Pap smears), contraceptive counseling, sexual health advice, and menopause management. Provide compassionate and confidential care to address the unique healthcare needs of women across the lifespan.
Chronic Disease Management: Support individuals with chronic diseases, such as asthma, COPD, and cardiovascular conditions, through comprehensive care planning, regular monitoring, and patient education. Promote adherence to treatment plans and lifestyle modifications to optimize disease control and improve quality of life.
Where You Will Work:
As a General Practice Nurse, you will work within General Practice (GP) surgeries as part of a collaborative Primary Healthcare Team. You may also work in community health centers, outpatient clinics, or mobile health units, depending on the healthcare setting and service delivery model in your region.
