Responsibilities:
- A qualified practitioner with the ability to manage caseload within a community setting with support and supervision. Assessment of new referrals to service. Liaising with clients, carers, and families via telephone, video conferencing, or face-to-face. Covering Duty. Taking part in regular MDTs.
Skills / Experience Required:
- Experience working within a community older adult or Memory Service team.
- The qualified mental health practitioner who knows about working with and managing a caseload autonomously in the community.
